Sink or Swim? How to succeed in a Value-Based Care World

As, staffing challenges and mounting regulations continue to plague post-acute and long-term care (PALTC) facilities nationwide, it is imperative to begin developing innovative strategies to adopt value-based care initiatives that will improve facility performance, occupancy, and revenues.
During this presentation, we will explore the various types of value-based programs available to facilities and proven strategies to improve facility performance. This session also shares best practices on how accessing live data analytics, pulled directly from the facility EHR, can reduce clinician burnout and administrative burdens by prioritizing clinical care, improving quality outcomes, lowering avoidable readmissions, and managing length of stay. Improving these key metrics enables facilities to demonstrate value as high-performing providers for their hospital and health plan partners thereby increasing referrals and boosting revenues.
