Leverage post-acute analytics to better manage patient care and become stronger partners for residents, referral sources, payers, and communities served. We’ll explore how Interventional Analytics helps reduce hospital readmissions, risk stratifies and clinically prioritizes patients, and advances care coordination with acute partners to gain referrals. We will also discuss the administrative and clinical impact data analytics can have on Infection Prevention and Control programs and address how analytics helps ensure appropriate payment for care provided.
