Incentivizing Your Post-Acute Care Network to Meet Contract Metrics

As Medicare enrollment expands to nearly 80 million beneficiaries by 2030, with 70% of those people expected to require long-term care and 40% needing skilled nursing care, it’s imperative not to ignore the significant and ongoing increase in demand for post-acute care. Being one of the costliest segments of healthcare, it is critical for ACOs to begin developing incentivized strategies to partner with their Post-Acute Network (PAN) in order to meet contract metrics.

This session explores proven strategies ACOs can implement to engage their PAN in driving care outcomes to meet specific value-based contract goals. Participants will learn how live post-acute data transparency goes beyond ADT and claims data to deliver the live clinical insights needed, at both the patient and facility level, to successfully guide and manage their PAN. We will demonstrate how these key insights enable ACOs to obtain regular reporting on essential clinical and financial metrics that can be used to develop shared savings opportunities for post-acute partners, motivating them to drive engagement and performance in meeting contract goals.
