SUMMARY:Incentivizing Your Post-Acute Care Network to Meet Contract Metrics
DESCRIPTION:As Medicare enrollment expands to nearly 80 million beneficiaries by 2030\, with 70% of those people expected to require long-term care and 40% needing skilled nursing care\, it’s imperative not to ignore the significant and ongoing increase in demand for post-acute care. Being one of the costliest segments of healthcare\, it is critical for ACOs to begin developing incentivized strategies to partner with their Post-Acute Network (PAN) in order to meet contract metrics. \nThis session explores proven strategies ACOs can implement to engage their PAN in driving care outcomes to meet specific value-based contract goals.
