Drive Facility-wide Quality of Care Improvements through Post-Acute Analytics

As value-based care models (VBC) continue to evolve, ACOs, hospitals and payers are expecting their post-acute partners to deliver better clinical and financial outcomes. This can put added pressure on Skilled Nursing Facilities (SNFs) in their efforts to improve quality measures, operate efficiently, meet regulatory requirements, and retain staff. To remain an essential provider and earn steady referrals, SNFs must adopt a VBC mindset across their entire organization. Technology is key to achieving this goal – in the form of data analytics.
