SUMMARY:Drive Facility-wide Quality of Care Improvements through Post-Acute Analytics
DESCRIPTION:As value-based care models (VBC) continue to evolve\, ACOs\, hospitals and payers are expecting their post-acute partners to deliver better clinical and financial outcomes. This can put added pressure on Skilled Nursing Facilities (SNFs) in their efforts to improve quality measures\, operate efficiently\, meet regulatory requirements\, and retain staff. To remain an essential provider and earn steady referrals\, SNFs must adopt a VBC mindset across their entire organization. Technology is key to achieving this goal – in the form of data analytics. \nThis session highlights how CHR Consulting Services leverages technology to better direct patient care\, streamline clinical workflow\, advance care coordination efforts\, and improve patient outcomes among the wide range of long-term care facilities they manage. We will explore the benefits of providing post-acute data transparency to their acute-care partners\, and discuss how CHR uses post-acute data analytics to help: \n\nImprove Quality of Care\nOptimize Customer Service\nIncrease and Maintain Census\nReduce Length of Stay\nAchieve Controlled Risk Management\n\nFirst-hand insights will be shared on tools and techniques used to easily access the live patient data already found within facility EHRs to risk stratify and clinically prioritize patients\, reduce readmissions\, and improve outcomes. Tricia Whaley\, Senior Director Provider Relations at CHR Consulting Services\, will discuss how they monitor and improve enterprise-wide performance with a complete picture of facility metrics in real-time\, including quality measures\, readmission rates\, PDPM reimbursement\, and infection prevention and control. \nBy using meaningful data to better manage patient care\, SNFs can become stronger partners for their residents\, referral sources\, and the communities they serve. \nLearning Objectives\n\nUnderstand the benefits of post-acute data transparency to achieve measurable performance goals and improve patient care\nIdentify key facility performance metrics to monitor clinical improvements and reduce costs at both the patient and facility level\nLearn best practices and strategies to reduce readmissions\, mitigate risk and increase hospital referrals\n\nSpeakers\n\nCheryl Scalzo\, RN\, Clinical Account Specialist\, Real Time Medical Systems\nTricia Whaley\, Senior Director Provider Relations\, CHR Consulting Services\n\nThis session is part of the 2023 PHCA Annual Convention & Tradeshow.
