Dr. Scott Rifkin Discusses the Vital Role Nursing Facilities Play in Value-Based Care
Scott Rifkin, MD, is an innovative healthcare leader with a passion for improving the health and lives of vulnerable seniors across the United States. With over 30 years of experience as an internal medicine physician, a significant part of his career has involved helping nursing homes deliver better, more efficient care. Scott has employed his knowledge and diverse skill set to start and lead companies committed to ensuring high-quality, value-based care for post-acute care patients.
From 2003 to 2017, Scott owned and operated Mid-Atlantic Healthcare. Under his leadership, the organization grew from one facility to 21 facilities with more than 4,000 beds throughout the Mid-Atlantic region. As a nursing home operator, Scott discovered that the inability to obtain meaningful clinical insights from electronic health records (EHRs) was an industry-wide challenge keeping long-term care facilities from delivering the best care possible. To solve this problem, Scott developed a proprietary platform that mines real-time patient data from the EHR to identify “interventional moments” in a patient’s care.
This ground-breaking platform became Real Time Medical Systems, which he founded in 2014. The company has become a leading data analytics platform used by post-acute care organizations across the U.S. to gain real-time, actionable insights for improving quality of care, clinical outcomes, and performance under value-based care models.
In 2015, Scott started Provider Partners Health Plans (PPHP) with a mission to “partner with providers to deliver excellent, compassionate and personalized health care for seniors.” PPHP offers Medicare Advantage Special Needs Plans in five states and helps post-acute providers improve care for vulnerable beneficiaries residing in long-term care facilities or who live at home and need the level of care provided in a nursing home.
