As value-based care programs become the norm, the need for acute and post-acute care (PAC) collaboration is greater than ever. With a shared goal of improving patient care and reducing costs, a true collaborative effort can be achieved through data transparency. Though the disparity between acute and post-acute Electronic Health Records (EHRs) makes the sharing of live data among care partners seem impossible, innovative technologies are able to bridge the gap and help drive care coordination efforts.
This session will show attendees how to create a successful PAC network, drive clinical performance, reduce costs, and improve the patient care continuum by leveraging data transparency. Join us for first-hand insights, best practices, and lessons learned on the role live data has played in improving quality of care, risk stratifying patients for clinical prioritization, reducing readmissions, managing length-of-stay, and improving overall care at both the network and patient levels.
