As CMS pushes for all Medicare/Medicaid patients to be enrolled in a Value-Based program by 2030, nursing facility care teams play a critical role in driving quality outcomes and financial success. This session explores how PALTCmed members can leverage live post-acute analytics to improve chronic disease management, reduce rehospitalizations, and enhance quality of life, while also impacting Case Mix Index (CMI) and reimbursement. Practical strategies and real-world examples will be shared to optimize care delivery, operational efficiency, and VBC efforts.
