Stop Guessing! Improve Enterprise Performance through Post–Acute Analytics

Post-acute care providers, especially Skilled Nursing Facilities (SNFs), are facing more challenges than ever before. Over the past two years, they’ve transformed care delivery under PDPM then had to re-evaluate almost every aspect of operations during the pandemic. COVID-19 further exacerbated staffing shortages, created more dependency on agency staff, impacted occupancy, and changed the very nature of SNF stays due to higher acuity patients.

As value-based care models (VBC) continue to evolve, ACOs, hospitals and other payers expect their post-acute network partners to deliver better clinical and financial outcomes. This puts SNFs under more scrutiny in their efforts to gain referrals, boost occupancy, operate efficiently, meet regulatory requirements, and retain staff. To remain an essential provider and earn steady referrals, SNFs must adopt a VBC mindset across their entire organization. Technology is key to achieving this goal – in the form of data analytics. This session discusses how multi-chain operator Legacy Health Services leverages live post-acute analytics to better manage patient care, streamline clinical workflow, and improve outcomes among its SNFs.

We will explore an effective, replicable strategy to reach these goals through post-acute data transparency and discuss how SNFs can easily access the live patient data already found within the EHR to risk stratify and clinically prioritize patients, reduce readmissions, and improve outcomes. Legacy will discuss how it monitors and improves performance enterprise-wide with a complete picture of facility metrics in real-time, including quality measures, readmission rates, PDPM reimbursement, and infection prevention and control. By using meaningful data to better manage patient care, SNFs can become stronger partners for their residents, referral sources, and the communities they serve.
