Managing Quality and Patient Outcomes in Your Short Stay Post-Acute Population

While the post-acute population may already be small in your facility, it accounts for significant revenue. With the emergence of “hospital-at-home” programs and home health agencies gaining popularity, competition for the short-stay patient has become greater than ever. Likewise, as CMS continues to drive bundled payment models to improve quality and control costs, many hospitals are building preferred care networks, with fewer Skilled Nursing Facilities (SNFs). So, what can you do to ensure the best possible outcomes for these short-stay patients, and become the post-acute option of choice for hospitals, health systems, ACOs and payers in your community?

Showing your acute and value-based care partners meaningful quality outcomes can drive referrals to new heights. Knowing what quality metrics are important to your partners and how to get great outcomes are key to having a healthy bottom line. During this session we will explore the metrics that most affect the short-stay patient and will discuss strategies facilities can implement to use this data to improve performance and reduce avoidable hospital readmissions.
