Sink or Swim? How to succeed in a Value-Based Care World

As CMS aims to have all Medicare beneficiaries and most Medicaid beneficiaries enrolled in a value-based care program by 2030, it is imperative for nursing facilities to begin developing innovative strategies to adopt value-based care initiatives that will improve facility performance, occupancy, and revenues.
During this presentation, experts will discuss the various types of value-based programs available to facilities and proven strategies to improve facility performance. Session speakers will also share best practices on how accessing live data analytics, pulled directly from the facility EHR, can reduce clinician burnout and administrative burdens by prioritizing clinical care, improving quality outcomes, lowering avoidable readmissions, and managing length of stay. Improving these key metrics enables facilities to demonstrate value as high-performing providers for their hospital and health plan partners thereby increasing referrals, boosting revenues, and succeeding in value-based care initiatives.
