Leverage Post-Acute Analytics to Inform and Improve Resident Care

Learn how resident care can be improved by leveraging post-acute analytics and become stronger partners for residents, referral sources, payers, and communities served. This session will explore how Interventional Analytics helps reduce hospital readmissions, risk stratifies and clinically prioritizes residents and advances care coordination with acute partners to gain referrals.
