The MDS Roadmap to Critical Components of Improved Clinical & Financial Outcomes

The Minimum Data Set (MDS) was created by CMS as a powerful tool for employing comprehensive evaluations and facilitating care management of Medicare-certified Skilled Nursing Facilities. With implications that affect patients, families, providers, and policymakers, recent updates to the MDS are designed to improve the reliability, accuracy, and effectiveness of the formulation and implementation of an individual’s care plan. With many new changes already rolling out or on the horizon, it’s important to note the profound impact the MDS has on patient care, public policy, and overall success of a facility. When completed, the MDS will trigger CAA’s and calculate Quality Measures, Quality Reporting Program data, value-based purchasing data, PDPM Case Mix index, some State Case Mix, as well as impact state survey efforts. That’s a lot of dots to connect!

As the MDS continues to evolve and impact reimbursement, this session will explore how post-acute facilities can utilize live data to better manage these connections while driving improved patient outcomes. We will explore how data analytics is helping facilities improve patient care while ensuring compliance within reimbursement and payment models. Participants will learn how proper utilization of data-driven technologies can be leveraged to identify, track and manage trends and insights, including clinical system breakdowns or MDS coding opportunities, and allow for appropriate MDS assessment and reimbursement. Furthermore, we will discuss how this approach enables SNFs to pull Interdisciplinary teams together, connecting the dots from the MDS to these critical components of the post-acute reimbursement and alternate payment models.
