Driving Better Patient Care and Outcomes With the CRISP Nursing Facility Connectivity Program

With a renewed focus on improving patient outcomes and driving better quality of care, the Chesapeake Regional Information System for our Patients (CRISP) assembled a statewide information technological initiative, specifically created to connect and integrate Skilled Nursing Facilities (SNFs). As the State-Designated Health Information Exchange (HIE) for Maryland, CRISP facilitates the sharing of data in real-time to healthcare providers in support of clinical decision making and driving patient care and outcomes.
In recognizing the challenges nursing facilities face every day, CRISP launched, with funding from the Maryland Department of Health, the Nursing Facility Connectivity Program in August 2022 to provide SNFs within the State of Maryland with the tools, resources, and Clinical Technical Assistance to better align with different provider care types. As the state’s HIE, it is crucial for care partners to seamlessly share data on patients across the care continuum to improve healthcare quality and outcomes while managing medications appropriately.
Hear from CRISP and Real Time Medical Systems (Real Time) as they discuss their partnership on this program, the importance of collaboration with the HIE, and how with the use of shared data, providers can improve care, lower healthcare costs, reduce hospital readmissions, and decrease preventable hospital admissions. Attendees will also learn how the program’s Clinical Technical Assistance team of nurses partner with facilities to provide an “extra set of eyes” on patients helping to prioritize patient care, identify care trends, and improve care transitions – with no additional work or duplicate data entry needed.
