Align Value-Based Programs with your Post-Acute Network to Ensure System-Wide Quality

In a value-based care world, ACOs, Hospitals and Health Systems are accountable for the entire patient journey; therefore, their long-term success and financial viability are inherently tied to the performance of their network of post-acute care (PAC) providers. A significant amount of PAC costs for ACOs and CMS’s Bundled Payments for Care Improvement initiative come from skilled nursing facilities (SNFs), who, thanks to the Patient Driven Payment Model, have begun adopting a value-based mindset to improve the quality of post-acute care. Now is the time to align acute and post-acute value-based care initiatives to deliver better clinical and financial outcomes across the entire patient continuum.
To realize meaningful reductions in healthcare costs and ensure system-wide quality, healthcare organizations must establish clinical care pathways that standardize processes and reduce variability within the PAC network.
