Align Value-Based Programs with your Post-Acute Network to Ensure System-Wide Quality

In a value-based care world, ACOs, Hospitals and Health Systems are accountable for the entire patient journey; therefore, their long-term success and financial viability are inherently tied to the performance of their network of post-acute care (PAC) providers.
