Interventional Analytics in Skilled Nursing Facilities Associated with Reduced Readmissions

This study evaluates the impact of an interventional analytics (IA) platform on 30-day risk-adjusted readmission rates in skilled nursing facilities (SNFs) affiliated with Penn Medicine’s Lancaster General Hospital (LGH). Using retrospective longitudinal data from CMS (2017–2022) and cross-sectional quality metrics, SNFs were grouped by location, referral relationship, and IA implementation status. Results showed that LGH-affiliated SNFs using IA had significantly lower readmission rates and better quality metrics compared to LGH SNFs without IA, other Pennsylvania SNFs, and national averages. The findings suggest that IA platforms may play a critical role in improving SNF performance and patient outcomes.
