Improve Outcomes and Gain Referrals as a High-Performance Post-Acute Provider

To perform well under bundled and other value-based payment models, ACOs and hospitals are inherently tied to the ability of their post-acute care partners to improve clinical and financial outcomes – namely by reducing avoidable hospital readmissions and managing length of stay in the post-acute setting. In an effort to provide improved patient experiences and generate savings, it is becoming common practice for acute care providers to keep a network of high-performing SNFs on call. So, which facilities make the cut and why?
This session explores what quality measures and metrics ACOs, hospitals, and health systems use to make referral decisions when sending their patients into post-acute care. In addition to lowering length of stay and reducing rehospitalizations, we will discuss how successful SNFs are utilizing live post-acute analytics to be better partners with their acute-care counterparts, offsetting the potential of lower revenue by driving referrals and ultimately increasing census.
