As you prepare to attend the upcoming PHW Kick-Off: Real Time Portal & Quality Overview Training, we want to ensure your team is ready to access the Real Time portal.

Before training begins, please complete the below credential request form for each user at your facility who will need access to the Real Time portal.

Once submitted, each staff member will receive an email from Real Time within 24–72 hours with their login instructions. Please note: Temporary passwords expire after 72 hours, so we recommend logging in as soon as possible after receiving the email.
