Nearly one-quarter of those admitted to SNFs are readmitted to the hospital within 30 days. With CMS seeking to improve patient outcomes through the SNF Value-Based Purchasing (VBP) program which provides financial incentives or penalties based on the 30-day hospital readmission rates\, it is crucial for post-acute providers to have low baseline rehospitalization rates and/or improvements over time. \nThe impact of rehospitalization is not only costly\, but negatively impacts our resident’s quality of life.  In addition to transfer trauma\, resident mortality rates double for those readmitted to acute care within 30 days and are four times more likely within 100 days of readmission. \nDuring this presentation\, we will discuss the impact of hospital readmissions on patient quality of care\, quality measures\, and reimbursement. We will highlight key strategies SNFs can implement to avoid sending patients back to the hospital and demonstrate how the use of live data analytics can support these efforts without adding extra administrative burden to care teams. Participants will learn best practices on how to utilize live data found within their EHR to risk stratify and clinically prioritize patients\, as well as take a proactive approach to care through clinical alerts and interventions. \nLearning Objectives\n\nUnderstand the impact of hospital readmissions on post-acute care\nRecognize best practices and strategies to reduce hospital readmissions\nDescribe how live post-acute analytics can identify quality improvement activities and be incorporated into daily clinical operations to reduce hospital readmissions\n\nSpeaker\n\nAngela Huffman\, RN\, WCC\, CHC\, Clinical Program Specialist\, Real Time Medical Systems\n\nThis session will be presented at the PADONA 36th Annual Conference.  URL:https://realtimemed.com/event/best-practices-strategies-tools-to-avoid-skilled-nursing-facility-snf-rehospitalizations/ LOCATION:Hershey\, PA CATEGORIES:Educational Session ATTACH;FMTTYPE=image/png:https://realtimemed.com/wp-content/uploads/2023/12/2024.04-PADONA.png END:VEVENT BEGIN:VEVENT DTSTART;VALUE=DATE:20240403 DTEND;VALUE=DATE:20240405 DTSTAMP:20240328T220043 CREATED:20231214T201935Z LAST-MODIFIED:20231214T202955Z UID:10000801-1712102400-1712275199@realtimemed.com SUMMARY:Healthcare Provider Transformational Assembly DESCRIPTION:Join us at the Healthcare Provider Transformational Assembly hosted by The Millennium Alliance. But what exactly does it mean to “be compliant?” It’s much broader than just following standards laid out by regulators – it’s also adherence to an organization’s internal protocols\, clinical standards\, and expected outcomes for various stakeholders. Compliancy touches all the steps that best guide care decisions and when combined with a Culture of Care and excellent Communication\, SNFs can achieve greater clinical\, operational\, and financial success. \nIn these challenging times\, as CMS sends out state and federal surveyors to ensure SNFs are providing safe\, quality care\, they are making sure care strategies are not just implemented and forgotten\, but that they are continually monitored and evaluated for success. This session explores how live data analytics can be leveraged by SNF teams to reduce administrative and regulatory burden by providing a means to monitor resident clinical status\, risk stratify patients\, and simplify communications facility-wide. We will discuss how data that already exists in facility EHRs can be used to help meet state and federal requirements\, prepare for regulatory reporting and onsite surveys\, conduct infection surveillance\, and mitigate risk. One of the most pivotal changes is an increase to the SNF QRP data completion threshold from 80% to 90% within the Minimum Data Set (MDS). \nIn order to ensure compliance\, SNFs must use the MDS to gather data on a minimum of 90% of assessments that are submitted to CMS. The penalty assessed to the SNF will entail a 2% reduction in their annual payment for fiscal year 2026\, resulting in decreased reimbursements. With the onset of these quality measure modifications in various stages of roll out\, it is critical for SNFs to be proactive in managing their data. \nAs the QRP continues to evolve and affect reimbursement\, this session will explore how live data analytics helps Post-Acute Care facilities better manage patient care/outcomes while ensuring compliance with the QRP measures. Learn how through early adoption and proper utilization of data-driven technologies\, Aperion Care identifies trends\, such as clinical system breakdowns or MDS coding opportunities\, as well as tracks and manages clinical trends and insights – allowing for appropriate assessment and reimbursement. \nLearning Objectives \n\nUnderstand the key documentation changes to the QRP\, including new data reporting standards and corresponding penalties\nLearn how live data analytics can help drive patient care and while improving outcomes\nEnsure facility compliance in meeting specified measures by integrating live data into the workflow\n\nSpeakers\n\nJennifer Roberts\, RN\, RAC-CTA\, Chief Reimbursement Officer\, Aperion Care\nMichele Self\, MA\, CCC SLP\, CMAC\, Clinical Account Specialist\, Real Time Medical Systems\n\nThis session will be presented at the AAPACN 2024 Conference.  URL:https://realtimemed.com/event/stay-ahead-of-evolving-qrp-requirements-through-data-analytics/ LOCATION:Hollywood\, FL CATEGORIES:Educational Session ATTACH;FMTTYPE=image/png:https://realtimemed.com/wp-content/uploads/2023/11/2024.04-AAPACN-1.png END:VEVENT BEGIN:VEVENT DTSTART;TZID=America/New_York:20240423T083000 DTEND;TZID=America/New_York:20240423T213000 DTSTAMP:20240328T220043 CREATED:20240309T011751Z LAST-MODIFIED:20240309T013823Z UID:10000816-1713861000-1713907800@realtimemed.com SUMMARY:Getting Back to Basics: Maintaining Regulatory Compliance DESCRIPTION:Maintaining nursing home regulations and compliance can overwhelm even the most prepared facility – and that task has become more daunting since the final Rules of Participation (RoP) mandates were put into action. Care providers were forced to examine policy and procedures\, while ensuring all components of regulations were being completed. Thanks to an unprecedented staffing crisis\, providers are scrambling to adhere to rapidly evolving guidance. To comply with CMS regulations moving forward\, operators must “get back to the basics” in preparing for compliance surveys and other state and federal rules. Facilities must be ready to show surveyors how they identify\, respond to\, and mitigate different types of risk. \nEstablishing a culture of compliance requires organizational leadership; facilities must have competent awareness of the state surveyors’ expectations and remain apprised of upcoming regulatory changes. This session explores how nursing operators can leverage live post-acute analytics\, pulled directly from EHRs\, to meet state and federal requirements\, prepare for regulatory reporting and onsite surveys\, conduct infection surveillance\, and mitigate risk. Highlighting the seven key areas of compliance associated with the surveyors’ process – from offsite survey preparation to the exit conference – this presentation highlights the key\, automated data points and processes SNFs can use to demonstrate compliance. The session will also explore best practices on how to collect and use live data within the EHR to evaluate the quality of care being provided to patients\, as well as to develop and monitor facility-wide performance improvement processes. \nLearning Objectives\n\nUnderstand how post-acute analytics can be incorporated into daily clinical processes and operations to maintain regulatory compliance\nEstablish live data-driven standardized care pathways to prioritize care and reduce administrative burdens\, while addressing state and federal requirements\nIdentify quality improvement practices to enhance care\, achieve compliance\, and better prepare for surveys\n\nSpeaker\n\nKathy Derleth\, RN\, BSN\, Clinical Program Director\, Real Time Medical Systems\n\nThis session will be presented at the South Carolina Health Care Association (SCHCA) 2024 Annual Convention. In an effort to provide improved patient experiences and generate savings\, it is becoming common practice for acute care providers to keep a network of high-performing SNFs on call. So\, which facilities make the cut and why? \nThis session explores what quality measures and metrics ACOs\, hospitals\, and health systems use to make referral decisions when sending their patients into post-acute care. In addition to lowering length of stay and reducing rehospitalizations\, we will discuss how successful SNFs are utilizing live post-acute analytics to be better partners with their acute-care counterparts\, offsetting the potential of lower revenue by driving referrals and ultimately increasing census. \nLearning Objectives\n\nUncover the quality measures vital to ACOs and hospitals that drive the referral process\nExplain what hospitals want out of their preferred post-acute network partners\nIdentify metrics to validate performance in conversations with upstream referral sources\nEstablish top five processes that high-referral SNFs are implementing to increase census\n\nSpeaker\n\nKathy Derleth\, RN\, BSN\, Clinical Program Director\, Real Time Medical Systems\n\nThis session will be presented at The 2024 New England Alliance Spring Regional Conference. Reports also show that antibiotics are among the most frequently prescribed medication in facilities\, with 6–10% of residents receiving antibiotics on any given day. However\, 40–75% of those prescribed antibiotics may be unnecessary or inappropriate – causing risk of serious complications\, development of serious gastrointestinal infections (i.e.\, Clostridium difficile)\, increased adverse drug events\, growth of multi drug resistant organisms\, and/or even death. HAIs also carry a huge financial burden\, costing the healthcare industry upwards of $2 billion annually for hospitalizations. \nAlthough daily surveillance and tracking of infections and antibiotic use in Skilled Nursing Facilities has traditionally been an arduous and timely process\, this session explores how technology can provide timely surveillance and identify early warning signs of HAIs\, highlighting the need for possible testing to determine the appropriate pharmacological and non-pharmacological interventions – resulting in avoided hospitalizations. We will also discuss how by automating the tracking and reporting of infections and antibiotic use facility-wide\, operators can leverage live-data analytics to meet state and federal requirements\, prepare for regulatory reporting\, and improve survey outcomes. \nLearning Objectives:\n\nIdentify strategies and best practices to leverage technology and data analytics to strengthen Infection Prevention and Control efforts\nManage efficient regulatory reporting and onsite surveys through early identification of HAIs\, proactive interventions\, and avoiding HAI-related hospitalizations\nIllustrate how live data analytics can be utilized to help reduce regulatory burden and improve HAI reporting outcomes\n\nSpeaker:\n\nCheryl Scalzo\, RN\, Clinical Account Specialist\, Real Time Medical Systems\n\nThis session will be presented at the NADONA LTC 37th National Conference.  URL:https://realtimemed.com/event/reduce-healthcare-associated-infections-hais-and-improve-patient-outcomes-through-technology/ LOCATION:Kansas City\, MO CATEGORIES:Educational Session ATTACH;FMTTYPE=image/png:https://realtimemed.com/wp-content/uploads/2023/11/2024.06-NADONA.png END:VEVENT BEGIN:VEVENT DTSTART;TZID=America/New_York:20240626T121500 DTEND;TZID=America/New_York:20240626T131500 DTSTAMP:20240328T220043 CREATED:20240315T185929Z LAST-MODIFIED:20240315T190049Z UID:10000818-1719404100-1719407700@realtimemed.com SUMMARY:Are Your Infection Prevention and Control Programs Survey Ready? Let Live Data Analytics Guide You DESCRIPTION:To ensure adequate care is being provided to the more than 1.2 million people who reside in one of the 15\,000+ certified nursing homes across the country\, CMS continues to remain diligent in its oversight of Skilled Nursing Facilities (SNFs). During the COVID-19 pandemic\, SNF regulations and surveys looked very different than what it is today. While the processes may vary across states and nursing facilities\, poor survey outcomes bring challenging consequences to all\, including potential civil financial penalties\, payment denials\, decreased referrals\, and declines in reputation. \nHistorically\, Infection Control related deficiencies have consistently been among the top 3\, and are now the top 2 cited deficient regulations. During this session\, we will discuss how data-driven technology can help ensure facilities are survey ready and in compliance with outlined federal regulations of F880\, F881\, F882\, and F883. Participants will also learn how taking a proactive approach to the appropriate utilization of data analytics can assist SNFs in identifying areas to enhance performance improvement plans and offer interventional measures. Additionally\, we will explore the reduced administrative and regulatory burden that comes with using live data analytics to reach and maintain compliance – enabling facilities to always be “survey ready.” \nLearning Objectives\n\nUnderstand Infection Prevention & Control regulatory requirements\nLearn how data-driven technology helps facilities maintain regulatory compliance\nUtilize live data analytics to assist in improving the quality of care and clinical outcomes facility-wide\n\nSpeaker\n\nCheryl Scalzo\, RN\, Clinical Account Specialist\, Real Time Medical Systems\n\nThis session will be presented at the NADONA LTC 37th National Conference.  URL:https://realtimemed.com/event/are-your-ipc-programs-survey-ready-let-live-data-analytics-guide-you/ LOCATION:Kansas City\, MO CATEGORIES:Educational Session ATTACH;FMTTYPE=image/png:https://realtimemed.com/wp-content/uploads/2024/03/2024.06-NADONA-2.png END:VEVENT END:VCALENDAR
