DESCRIPTION:The COVID-19 pandemic has exposed a lot of inefficiencies within healthcare — one of the largest being the ability to share and access patient information from your post-acute networks. \nOne of the riskiest times for misinformation to occur is when a patient is discharged from the hospital and placed into another care setting\, such as a skilled nursing facility (SNF). This is especially true for patients age 60 or older who are at higher risk for infections\, present more complex health needs\, and have proven to be at higher risk of rehospitalization. Though the disparity of post-acute EHRs makes the sharing of live data among care partners seem impossible\, innovative technologies are able to bridge the gap and help drive care transition efforts. \nThis session takes an in-depth look at how ACOs can impact earned shared savings though post-acute data transparency.
