DESCRIPTION:In today’s healthcare landscape\, focus on achieving value-based care (VBC) outcomes has never been more critical. For Accountable Care Organizations (ACOs) and Payers\, an important key to success in meeting VBC goals is to prioritize the development of robust Skilled Nursing Facility (SNF) networks. However\, creating a network that merely exists isn’t enough—ineffective networks lacking efficacy must be avoided. This session delves into strategies for building and maintaining a high-performing SNF network that leverages live data analytics to drive cost savings and improve patient outcomes. \nWe’ll examine the impact SNF performance has on ACOs and Payers and explore the benefits of establishing a high-performing network supported by data-driven decision-making and solutions that empower SNFs within the network.
