DESCRIPTION:Health plans today require more insight and influence across the care continuum. Given that post-acute care (PAC) is one of Medicare’s most utilized and costly segments\, with skilled nursing comprising about half of those costs\, establishing a strong PAC network is key to improving outcomes and reducing the total cost of care. But that’s difficult due to limited visibility into how care is delivered in skilled nursing facilities (SNFs). \nTo build a successful network and drive better outcomes\, you need relevant\, timely information about member health status during the SNF stay. Relying on outdated claims information does not yield the actionable insights necessary to effectively manage the post-acute episode of care.
