SUMMARY:Managing Your Post-Acute Spend - The Cost of Doing Nothing
DESCRIPTION:In a value-based care world\, ACO’s\, are accountable for the entire patient journey; therefore\, their long-term success and financial viability are inherently tied to the performance of their post-acute care (PAC) providers. As the ACO/PAC landscape begins to transform with many ACOs now building preferred PAC networks\, there is still a lot of work to be done in creating successful care coordination efforts that align with value-based care initiatives and incorporate effective care transitions\, clinical pathway standards of care\, risk stratification\, appropriate length of stays\, and reduced readmissions. \nThough the financial impact of this effort may seem minimal – do we really understand the risks and costs involved if we choose to do nothing with post-acute providers? In a recent study\, conducted by the Center for Health and Research Transformation (CHRT)\, it concluded that poorly coordinated care transitions from the hospital to other care settings cost an estimated $12 billion to $44 billion per year. This cost will only increase as baby boomers are estimated to double current Medicare beneficiaries by 2030 – demonstrating there is a true cost to inaction and at the forefront\, it is the patients that suffer. \nDuring this webinar\, Phyllis Wojtusik\, RN and Margie Latrella\, APN-C will discuss the clinical and financial impact of “doing nothing” when it comes to post-acute spend including: \n\nMissing out on quality of care\nNot knowing your network performance\nLosing potential shared savings dollars on PAC patients\n\nMargie and Phyllis will also highlight how live post-acute data analytics can help reduce total cost of care while enabling a better quality of care and achieving value-based outcomes. \nLearning Objectives\n\nUnderstand the financial and clinical impact “doing nothing” has as it pertains to post-acute spend\nImprove transitions of care by ensuring patients go to the right care setting at the right time\nImplement data-driven strategies to meet quality scores and increase earned shared savings while improving quality of care\n\nSpeakers:\n\nMargaret (Margie) Latrella\, Vice President\, Clinical and Network Quality\nPhyllis Wojtusik\, EVP\, Health System Solutions\, Real Time Medical Systems\n\nThis free live webinar event is hosted by NAACOS.
