SUMMARY:Managing Sepsis: Insights\, Analytics\, and Strategies for Nursing Facilities
DESCRIPTION:The burden of sepsis is high in the U.S.\, with over 1.7 million adult cases resulting in 350\,000 deaths reported annually. Patients who survive sepsis often suffer long-term physical\, psychological\, and cognitive disabilities. A leading cause of unplanned 30-day hospital readmissions\, experts believe as many as 80% of sepsis deaths could be prevented with early detection and treatment. With no singular diagnostic test to identify sepsis\, clinicians must rely on physical signs and symptoms along with evidence of infection and organ dysfunction to diagnosis this life threatening\, medical emergency. \nThis session explores how nursing facilities can leverage live data analytics to detect the risk of sepsis early\, allowing clinicians to intervene in care prior to an adverse event or need for hospitalization. Learn how Maryland’s designated Health Information Exchange (HIE)\, the Chesapeake Regional Information System for our Patients (CRISP)\, is helping facilities within the program reduce sepsis-related cases. By utilizing live alerts pulled from data that already exists in the resident’s EHR\, care teams are identifying the early onset of sepsis and providing interventional treatment before readmission is necessary.
