SUMMARY:Going Beyond Interoperability – The Data You Share (And How You Share It) Matters
DESCRIPTION:In March 2020\, the Centers for Medicare & Medicaid Services (CMS) and the Office of the National Coordinator for Healthcare Technology (ONC) released their latest updates to healthcare IT regulations\, which included final rules on interoperability. And while some in the post-acute care space have been reluctant to offer up their data to health care partners\, others are realizing the many advantages to providing post-acute interoperability. \nLike it or not\, with CMS planning to have all Traditional Medicare patients in an accountable care relationship by 2030\, it’s imperative nursing facilities get on board with the idea of granting post-acute data transparency to the appropriate partner providers to survive. \nThis session delivers the “why” and “how” facilities should share both patient and facility-level data with their acute care partners\, particularly those participating in value-based programs. We’ll uncover the key performance metrics facilities can leverage to elevate care coordination\, standardize care\, ensure appropriate transitions between care settings\, prevent avoidable hospital readmissions\, and most importantly\, improve patient outcomes. This session also explores strategies and techniques nursing facilities can implement to deliver post-acute interoperability in a way that reduces their administrative and regulatory burden.
