DESCRIPTION:Join us at the ACHE 2023 Congress on Healthcare Leadership as we take the stage with St. Joseph’s Health Partners to present: \nLower Value-Based Care Costs through Post-Acute Data Transparency\nIt is projected that by 2029\, more than 80 million Americans will be eligible for Medicare. With an estimated 10\,000 baby boomers entering the system every day\, is healthcare ready to manage this aging population that is living longer with more chronic illnesses? \nThis session will demonstrate how healthcare executives can create a successful post-acute network\, drive clinical performance measures\, reduce total cost of care and improve the patient care continuum by strengthening care coordination efforts with the help of data transparency. \nManager of Clinical Transformation of  St. Joseph’s Health Partners\, Lavana Baldasare\, BSW\, AAS\, RRT-NPS\, AE-C\, will discuss how St. Joseph’s Health achieved post-acute interoperability\, which led to improved patient outcomes in its community and a cost savings of $1.6 million in the first year of implementation. She will explore the role data transparency played in helping St. Joseph’s Health improve quality of care\, reduce readmissions\, and identify high-quality\, low-cost providers within its network. \nMs. Baldasare will be joined by Diana (“Di”) Smalley\, FACHE\, who has more than 30 years of senior-level experience in healthcare and a deep understanding of patient-driven health and operational innovations. Also lending her expertise to the conversation will be Real Time Medical Systems’ Executive Vice President of Health Systems Solutions\, Phyllis Wojtusik\, RN\, who\, prior to becoming an integral part of Real Time’s coordinated care effort\, established a successful post-acute care network for a 4-building hospital system in Pennsylvania. Rounding out the team to discuss first-hand experiences\, insights\, and lessons learned is Margaret (“Margie”) Latrella\, APN-C\, Vice President of Clinical and Network Quality for Real Time.
